ABSTRACT

Objective To discuss the short-term clinical curative effect of laparoscopic colorectal cancer radical resection for rectal cancer. Methods Clinical data of 50 patients with rectal cancer underwent radical resection from November 2015 to November 2016 were retrospectively analyzed. Among them, 27 cases underwent laparoscopic radical resection (Laparoscopy group), the other 23 cases underwent radical resection (Laparotomy group). Then observe and record the operation time, tumor diameter, specimen length, number of lymph node cleaning, time of ambulation, postoperative anal exhaust time, postoperative defecation time, postoperative complications and postoperative eating time of the two groups. Results The tumor diameter, length of specimens and number of lymph node dissection in laparoscopic group were (3.8 ± 1.4) cm, (18.5 ± 2.1) cm and (7.2 ± 3.1), while in Laparotomy group were (3.9 ± 1.4) cm, (18.6 ± 2.3) cm, and (7.7 ± 3.4), the difference has no statistical significance (P > 0.05). The intraoperative blood loss, operation time, ambulation time, postoperative anal exhaust time, postoperative defecation time, postoperative eating liquid diet time, postoperative hospitalization time in laparoscopic group were (105.3 ± 23.8) ml, (140.2 ± 22.3) min, (4.0 ± 1.2) d, (6.0 ± 1.5) d, (3.0 ± 1.0) d, (3.5 ± 0.5) d and (4.0 ± 1.0) d, while in Laparotomy group were (210.4 ± 21.3) ml, (118.9 ± 20.7) min, (4.5 ± 1.1) d, (7.8 ± 1.2) d, (7.0 ± 1.6) d, (8.1 ± 2.0) d and (10.0 ± 3.2) d, there was significant difference between the two groups (P < 0.05). Conclusion Laparoscopic surgery for rectal cancer is safe and effective. It can achieve radical tumor resection, and intraoperative less bleeding, faster postoperative recovery, shorter hospitalization time.

ABSTRACT

Objective To explore the safety of the laparoscopic surgery in radical resection for different stage rectal cancer. Methods Clinical data of 200 cases with rectal cancer underwent laparoscopic radical resection(observation group) were analyzed retrospectively, including 52 cases of stage 0-Ⅰ,148 cases of stage Ⅱ-Ⅲ.Two hundred cases patients with rectal cancer underwent open radical surgery were selected as control group, including 44 cases of stage 0-Ⅰ,156 cases of stage Ⅱ-Ⅲ.The length to distal resection margin, the number of harvested lymph nodes, the incidence of postoperative complications and the prognosis of recent situation were observed, the safety of the laparoscopic surgery was analyzed. Results Whether rectal cancer of stage 0-Ⅰ or Ⅱ-Ⅲ,there were no significant differences in terms of the length of distal resection margin((2.5±0.9) cmvs.(2.4±1.1) cm,t=0.490,P=0.625;(3.1±1.0) cm vs.(3.2±1.3) cm,t=0.749,P=0.454),the number of harvested lymph nodes((12.3±4.2) vs.(12.7±3.9),t=0.480,P=0.632;(13.9±5.4) vs.(15.1±4.9),t=2.369,P=0.118),the incidence of postoperative complications (17.3% vs.18.2%,x2 =0.012,P=0.911;27.7% vs.28.8%,x2=0.049,P=0.825),the rate of local recurrence(1.9%vs.2.3%,x2 =0.014,P =1.000;4.1% vs.3.2%,x2=0.157,P=0.692),the distant metastasis (3.8% vs.2.3%,x2=0.195,P=1.000;5.4% vs.4.5%,x2=0.137,P=0.712) and 3-year survival rate (96.2%vs.95.5%,x2=0.010,P=0.808;83.8% vs.85.3%,x2=0.132,P=0.714) between the observation group and control group (P>0.05).Conclusion For rectal cancer of stage 0-Ⅰ,laparoscopic surgery has good safety, and worthy of popularization and application. For rectal cancer of stage Ⅱ-Ⅲ,due to its operation for a long time as well as the possible risk in postoperative anastomotic fistula, the implementation of laparoscopic surgery should be more cautious.

ABSTRACT

Objective To evaluate the clinical significance of low ligation and high ligation of inferior mesenteric artery (IMA) and lymph nodes dissection on radical resection for rectal cancer. Methods One hundred and fifty-six patients who were diagnosed rectal cancer in our hospital between May 2007 and May 2008 were divided into low ligation group (80 cases)and high ligation group (76 cases). The low ligation group was treated with low ligation of IMA and lymph nodes dissection, the high ligation group was cured by high ligation of IMA and lymph nodes dissection.cases.The IMA lymph nodes metastasis, number of lymph nodes, cancer recurernce rate,5-year survival rate, complication rate were compared and analyzed. Results The rate of lymph nodes metastasis around the origin of inferior mesenteric artery was 15.0% in the low ligation group, the rate of lymph nodes metastasis around the origin of inferior mesenteric artery was 14.5% in the high ligation group, and the difference was not statistically significant (P > 0.05).Compared two groups of postoperative recurrence rate,5-year survival rate, anastomotic leakage rate, sexual dysfunction rate and urinary retention rate, there was no significant differences (P > 0.05).The intestinal function recovery time and low anterior resection syndrome incidence of the low ligation group were lower than the high ligation group, there were significant differences (P < 0.05).Conclusions Low ligation of inferior mesenteric artery and lymph nodes dissection can achieve radical resection for rectal cancer.Compared with traditional high ligation of inferior mesenteric artery, there were no differences for patients on recurrence rate,5-year survival rate and complication rate.
